Against: am unable to move gmt hand without moving the hour hand. The date mechanism works fine, but is difficult and fiddley to set.
Comes without GMT instructions. Had to go to other sites for this.

What is the 4th hand for?
How do I set the 4th hand ?

The GMT hand is functional. First I suggest you read the instructions provided with your watch. Although they are not specific to your watch they do give an overview.
With the dual time zone watches there is an extra position for the crown and an extra hand which shows GMT (Greenwich Mean Time). Basically you use the first position to set GMT time and then the second position to set the hour hand to your home time zone. You can then turn the dial to to select any time zone in the world.
It is a little complicated to explain in detail in an email so I recommend you watch this video.
